Certainly one of the worst was New York Times v. Sullivan. With that decision, the court gave the media license for irresponsibility. Quite often in recent years, a major news outlet has had to apologize for inaccuracies. Those apologies often come after the damage is done, and they never seem as "loud" as the original headline. But the important point is that they are repeat offenders; there is no deterrence... and no goad to accuracy.
